How to Make a Survey/Questionaire

  1. Decide what information you wish to collect - what did you find in your libarary research?
  2. Decide what type of survey, the type of response scale, how are you going to analyze the data?
  3. Write a draft - get your ideas down on paper
  4. Reexamime each question, revise or omit questions - the order of questions correct? (beliefs before behaviors)
  5. Pretest the survey on a few people to discover ambiguities and problems
  6. Revise the questions as needed - do they make sense to readers?
  7. Finalize the procedure you will use to collect data (e.g., sampling)

Be aware of social desirability

Consider the design and the order of questions
- use simple vocabulary
- avoid double barreled questions (are you dumb and stupid?)
- make questions brief
- avoid leading questions (normal folks think this, what do you think?
- avoid loaded questions (no emotionally charged words like dumb or stupid)
